Welcome
to Kindergarten! We look forward to working with all of you
to make this a happy and healthy year for your children. The
following information is intended to assist you throughout
the school year with medical issues that might arise, as well
as a few items that need to be addressed now. It might be
helpful to keep this information as a reference during the
coming year.
*EMERGENCY
CARDS: Need to be completed and returned by the first
day of school. This will give us ready access to you, as well
as the people you designate as emergency contacts in your
absence. We will always try to reach you, your spouse, or
significant other, first. If we are unable to reach you, we
will call the people on your list, in order. Please be sure
they are people who are able to pickup your children in your
absence, that they live in the general area and that you let
them know you have named them as emergency contacts. If you
have a regular daycare provider, make sure she/he is listed
on this card.
*IMMUNIZATION
RECORDS: In accordance with the laws of the state
of Massachusetts, NO CHILD WILL BE ADMITTED TO SCHOOL
WITHOUT PROOF OF FULL IMMUNIZATION. You MUST have
this information available BEFORE the first day of school.
Please note that as of 1999, all children entering kindergarten
MUST be immunized against chickenpox or be able to demonstrate
proof of history of the illness to their healthcare provider.
*PHYSICAL
EXAM RECORD:
Each child entering kindergarten must also have a physical
exam that is current. The deadline for submitting this is
the end of the current calendar year.
*MEDICATION
POLICY: The town of Shrewsbury has a very firm policy
that prohibits all elementary students from carrying any medication
on the school bus.If your child requires a daily medication
that must be given during school hours, you must provide us
with that medication in person, along with completed paperwork
from your health- care provider. All prescription medications
must be in pharmacy labelled containers- no loose pills in
baggies!!
*ILLNESS:
Always a difficult decision right before the school day starts.
A few suggestions: If your child has had a high temp., has
vomited or had diarrhea more that once the night before, consider
giving them the day off to allow them to recover. Keep in
mind that you know what is best for your child and this decision
should be yours, not theirs. Children who have a temp. over
100 degrees, have vomited or had diarrhea at school, will
be dismissed. Children with strep throat or conjunctivitis
(pink eye) must be on antibiotics for 24 hours before they
may return to school. If your child is injured on the playground
or in the building and has any injury to the head, we will
call you to make you aware, in all cases. We will not call
you for routine colds without fevers or for the many minor
bumps and bruises we experience.
*VISION
AND HEARING SCREENING: This will be done in the fall.
If we have any concerns, we will notify you. Please note that
this is only a screening and not meant to replace an examination
by your healthcare provider. Please feel free to call us at
any time with concerns or to update us about a change in your
child’s health.
