Registration Information
To register your child you will need:
1. Proof of Residency in Shrewsbury
One of the following is required:
a. Apartment Lease
b. Signed Purchase and Sales Agreement with date of occupancy
c. Utility bill (i.e., electric bill, tax bill, gas bill, Purchase & Sales agreement & Lease –
NO telephone bill, Bank statements, or driver’s license will be accepted)
2. Immunization Records & Copy of Last Physical- PDF
of Immunization Needed
Shrewsbury Public Schools is in the process of improving our security protocol
between your home computer and our server by encrypting your data submission.
When you click to begin in step #3, a security certificate will appear. Depending
upon the browser you are using, please select OK, Accept or Continue.
The certificate prompt may look like these two examples.

3. Complete Online Registration ( Click
to begin)
4. Student’s Social Security Number (Not required for
enrollment; however it is used for college transcripts. It can be submitted
electronic if you wish in step #3)
5. Student’s Birth Certificate or Passport-You must bring
in the birth certificate that has a seal
6. Custody Papers showing you have physical custody (if applicable)
7. Restraining Orders (if applicable)
8 . Student Transcripts (High School level) – Last Report
Card (Elementary and Middle School level) Student records will have to released
from prior school. Parents or Guardians will have to complete a Student Record
Release form. ( Download PDF Form ).
9. Massachusetts Transfer Card -If coming from a public school
within Massachusetts (You would get this form from the school you are coming
from)
10. Copy of IEP (Special Education Records) if applicable

Please bring all of the above to the
Office of the Superintendent of Schools at 100 Maple Avenue
(Directions
to Central Office)
Upon completion of the registration process, your child will be assigned to the appropriate school. Please note that children must be five years of age by August 31st of the year in which they enter Kindergarten and six years of age by August 31st of the year in which they are to enter Grade One. Students enrolling for Preschool must call 508-841-8646 to check if space is available and for age requirements before completing the registration form. Students over age 20 or who have completed the equivalent of a high school education will not be registered in the Shrewsbury Public Schools.
